Peer Review Workflow


ACTION & ACTIVITY


ALL librarians receive written notification indicating eligibility for peer review cycle.


Deadline for RIs seeking approval for an off-cycle action for a candidate not eligible for review. Request must be submitted via email to the UL with copies to Unit/Department Head, AUL and ADAHR.


Candidates provide Form 17 to RIs to communicate suggested letter writers and suggested exclusions (RI must provide internal deadline for this form).

RIs request letters of assessment on performance, if needed for action, as soon as recommended action is determined.

RIs and Department Heads meet with their Supervisory AUL to review pending actions and schedule. AULs should set internal deadlines for documentation to be submitted within their own divisions.


Candidates prepare and submit preliminary drafts of Statement of Responsibilities, Data Summary, and Statement of Professional Achievements to RI.

RIs schedule and hold preliminary assessment and evaluation conferences to discuss pending actions with candidates. RIs discuss actions with Unit Directors and/or AULs BEFORE final recommendation is made and communicated to candidate.

RIs and Librarians conduct final evaluation/recommendation discussions.

Candidates prepare final versions of Statement of Professional Responsibilities, Data Summary, Statement of Professional Achievements, and any other documentation.

RIs prepare final evaluation and recommendation.


Deadline for RI to submit Form #2-A to LHR to prompt initiation of case in Opus (candidate should be copied).

Deadline for individual librarians to request a deadline extension or a deferral review from the UL - request must be submitted via email to the UL with copies to RI, Department Head, AUL and ADAHR.


LHR initiates all cases in Opus, and Librarians under review are notified.

Review file in final form due to Supervisory AUL – unless an extension has been approved by the UL by the established deadline to request extension.

Deadline to submit Form # 3-A Candidate’s Certification Statement – Exclusions to the ADAHR. Send completed form in an email marked ‘confidential’ to liliavaldez@library.ucla.edu.

Form 3A is only required when the candidate is requesting LAUC-LA members to be excluded from participating in CAPA and/or ad hoc committees in the review of their file.


AUL submits (Form #8) and discusses their comments with the candidate, RI, and Department Head.


Candidate has at least seven (7) calendar days to review all comments and recommendations and prepare additional comments for their file (See Form 16 - Rejoinder, optional).


Opus case file due to LHR – unless an extension was approved by the UL by the established deadline.

The Librarian under review is responsible for uploading ALL final versions of documentation to Opus.


LHR reviews Opus case file for completeness and transmits files to CAPA as the review is completed. If files are found incomplete, LHR will return files to librarians under review.


CAPA meets to review Opus case files and prepares written recommendations to UL. Ad hoc committees set up by CAPA as needed. CAPA submits files to LHR as they are completed.

LHR reviews and forwards files to UL as they are completed.


UL reviews files, makes final decisions, and prepares letters.


All librarians notified of final decisions. (All salary actions are effective 07/01/2024, paid on 07/31/2024.)
