Request a Long-Term Loan or Windows Laptop

Use this form if you would like to request a long-term loan of a CLICC device MacBook, iPad, or HP Windows) for the current quarter because you are a:


  • Student enrolled in a UCLA registered course that requires specialized software to fulfill coursework OR
  • TA/Professor instructing a UCLA registered course where your department does not provide you the necessary equipment to fulfill the role.

(General software/programs like Notability, Good Notes, Microsoft Applications, Google Apps, etc are not considered specialized programs and would not be eligible for approval)

The following requirements are mandatory to submit for your request:

Undergraduate/Graduate Students Professor/Graduate TA/Guest Lecturer/Tutor
  1. Course Name currently enrolled in
  2. 9-Digit SRS Course Number (can be found on Schedule of Classes)
  3. Name(s) of Specialized Software needed to fulfill course work
  4. Proof of Enrollment (only unofficial transcript downloaded from myUCLA will be accepted)
  1. Course Name currently instructing
  2. 9-Digit SRS Course Number (can be found on Schedule of Classes)
  3. Name(s) of Specialized Software needed to fulfill role
  4. Proof of Instruction (attach TA or Tutor appointment letter)

If you cannot provide ALL of the above requirements, please kindly follow our regular lending policies of a 2-week loan period or utilize our CLICC Virtual Desktop to access a Windows operating system from your personal device.

The CLICC Fines & Fees Policy is applicable to all CLICC Equipment, including long-term loans.
